CHANGE OF STUDENT INFORMATION

If you need to make any changes on your student’s information, please complete the form and return it to the Registrar’s office.  Any address changes must be accompanied by a current land-based utility bill for verification of residency.  A copy of the parent’s driver’s license is also required.  You may use this form to change the address, phone numbers, add to the emergency contact/pick up list, or to remove someone from the emergency contact/pick up list. You can also log onto your Family Access account to update phones numbers and emergency contacts. Only parents or legal guardians can make these changes with ID/DL.
